	24 Hour Death and Serious Injury Report Link
	Senate Bill 1024 requires districts to report "serious bodily injury" or "death" of personnel due to a restraint or seclusion of a student to the Superintendent of Public Instruction. Districts must use this link to report the above within 24 hours of the incident.
